2026 FIFA World Cup Concussion Protocols: What Happens If a Player Suffers a Head Injury?

Posted on July 1, 2026July 2, 2026 by Santiago Leon

The 2026 FIFA World Cup concussion protocols are designed to protect players who suffer a head injury or show signs of a possible concussion during a match. FIFA follows a “Suspect and Protect” approach, meaning that if there is any doubt about a player’s condition, the player should be removed from play and assessed by medical staff.

What Are the Concussion Protocols at the 2026 FIFA World Cup?

If a player suffers a blow to the head, neck, face, or body and shows signs of a possible concussion, the referee can stop play so the team’s medical staff can evaluate the player. FIFA’s guidance says anyone with concussion symptoms should leave the pitch immediately and be checked by a doctor. (Inside FIFA)

Do Teams Get an Extra Substitution for Concussions?

Yes. Under IFAB’s additional permanent concussion substitution protocol, a team may use a concussion substitution that is separate from normal substitutions. This means the change does not count against the team’s regular substitution limit. (IFAB)

Can a Player Return After a Concussion Substitution?

No. Once a player is replaced because of a concussion or suspected concussion, that player cannot return to the match. IFAB also states that the player cannot take part in a penalty shootout. (IFAB)

Why Does the Opposing Team Get an Extra Substitution?

To avoid giving one team a competitive advantage, the opposing team is also allowed an additional substitution opportunity after a concussion substitution is used. This keeps the match fair while still prioritizing player safety.

Do Concussion Rules Apply in Extra Time?

Yes. The concussion protocol applies during regulation time, stoppage time, extra time, and knockout matches. If a player suffers a suspected concussion late in the game, the team can still use the concussion substitution if available.

Why the Rule Matters

Concussions can be serious brain injuries, and symptoms are not always obvious right away. The World Cup protocol is meant to encourage teams to remove players from danger instead of leaving them on the field because of substitution concerns.

2026 FIFA World Cup Concussion Protocol Summary

The 2026 FIFA World Cup concussion rules allow teams to remove a player with a suspected concussion and replace them without using a normal substitution. The injured player cannot return, and the opposing team receives an extra substitution opportunity for balance. FIFA’s main message is clear: no match is worth risking a player’s health.
